Job Title: Registered Nurse (RN) - Case Manager
Locations: Los Angeles, Irvine, San Diego, CA
Duration: 3 Months+ (Possible Extension)
 
Schedule: (10*4) - 40 hrs
Shift: Day 09:00 PM to 07:30 AM
Pay Rate: $60 -$72 per hour. on W2 & $3600 weekly for travellers
 
Job Summary:

• Under general Case manager, provides nursing care in a hospital to a variety of patients with health problems ranging from simple to complex.

 
Job Responsibilities:

• Inpatient CM and develop care plans accordingly.
• Facilitate communication between the interdisciplinary team, patients, and their families.
• Ensure compliance with JCAHO and hospital guidelines in care planning processes.
• Maintain communication with payers to secure necessary approvals for care
• Pursues self-development; creates plans and takes action to capitalize on strengths and develop weaknesses; influences others through technical explanations and examples.
• Adapts to and learns from change, challenges, and feedback; demonstrates flexibility in approaches to work; helps others adapt to new tasks and processes.
• Supports and responds to the needs of others to support a business outcome.
• Completes work assignments autonomously by applying up-to-date expertise in subject area to generate creative solutions; ensures all procedures and policies are followed; leverages an understanding of data and resources to support projects or initiatives.
• Collaborates cross-functionally to solve business problems; escalates issues or risks as appropriate; communicates progress and information.

Required Experience:

• 1 year of experience recent case manager with Epic.

 
Required Certifications:

• RN License
• BLS
• Epic EMR experience
