Job Duration: 2-3 Months (possibility of extension)
Shift: Monday - Friday, 8:30am - 5:00pm
Payrate: $44/ hr. on W2

Job Summary:
- Fulfills the needs of patients, families and community by providing Social Work services in accordance with his/her licensure, certification, training, and regulatory requirements.
- Assumes the responsibility for coordinating and facilitating services for patients during and subsequent to admission to the acute rehabilitation unit.
- Conducts discharge planning, coordination of care, and other planning and counseling activities to effectively move the patient through the continuum of care in a cost-effective and resource efficient manner.
- Responds effectively to new and changing work assignments.
Experience Required:
- At least 1 year of hospital case management (discharge planning) experience required.
- Knowledge of client.
Certifications Required:
-
LICSW, active DC SW license.
Other Requirements:

Must live 50 miles away.
