Note: We have opening for part time/ fulltime position with morning evening and overnight.
Job Description:
- Gives baths (partial or complete).
- Showers as scheduled and when necessary Minimum shower twice weekly.
- Shampoos hair at least weekly, combs and arranges resident's hair daily and when necessary.
- Shaves residents, including removal of facial hair of female residents, trims and cleans fingernails of residents on a weekly basis and when necessary.
- Provides daily oral hygiene and denture care each shift and when necessary.
- Empties and measures drainage (urine, tubes, vomitus, etc.) as ordered and reports amount, color, and consistency to nurse-in-charge.
- Gives incontinent and skin care to residents as is necessary.
- Dresses resident appropriately.
- Informs nurse of clothing needs and keeps drawers and closets neat and orderly.
- Encourages the resident to participate in his/her daily activities.
- Carries out bladder and bowel monitoring and training program as assigned and records results.
- Checks bowel movements daily and records results on NAAARS every shif; after three days, of no bowel movement notify the nurse.
- Assists nurse in dressing changes when necessary.
- Gives care and support to residents who are terminally ill, isolated, debilitated, or cognitively impaired.
- Assists residents from bed to chair, stretcher and vice versa with use of Mechanical Lift with mandatory two or more person assistance and buddy system assistance.
- Assists ambulatory residents to walk using walker or cane as indicated.
- Participate in resident floor ambulation program.
- Transports residents to clinic, activity programs and other appointments, including but not limited to appointments outside of the facility during working hours.
- Turns and position residents every 2-3 hours for support and comfort, in the prevention of skin injury and contracture.
- Provides range of motion to residents.
- Applies support stockings, and splints as directed by the nurse.
- Assists residents to undress and prepares residents for bed in accordance with residents' needs and abilities.
- Answers residents' call bells and alarms promptly.
- Positions side rails, as per protocol (Restraint Free environment).
- Observes and assists restless or cognitively impaired residents as directed.
- Observes any significant change in residents' behavior or physical condition and informs the nurse immediately of noted changes.
- Takes and records temperature (orally or rectally as indicated), pulse, respiration and blood pressure into the electronic medical record (EMR).
- Take and records weights as assigned.
- Prepares residents for meals in a pleasant environment with no distraction.
- Feeds or assists with feedings as needed.
- Provides fresh water and nourishment as directed.
- Encourages fluids as ordered. Measures and records intake and output of residents.(meal consumption sheet)
- Informs nurse in charge of inadequate intake and /or output.
- Assists in admissions, transfers, and discharges.
- Makes bed when indicated.
- Maintains clean and non-cluttered environment in residents' room and service areas.
- Observes and reports equipment needing repair.
- Practices economy in use of utilities and supplies.
- Documents care on accountability records and forms.
- Obtains report from nurse-in-charge regarding care delivered (per shift).
- Gives report to nurse-in-charge regarding care delivered (per shift).
- Performs other related duties as assigned by charge nurse, supervisor, Associate/Director of Nursing.
- In an emergency, performs other duties necessary for the care of the residents, or the efficient operation of the facility.
- Monitor residents assigned by nurse Q15 min and Q30 min.
Required Skills & Experience:
- Knowledge on the prevention of skin breakdown.
- Ability to communicate with residents in a compassionate and gentle manner, ability to understand and carry out verbal and written instructions.
Required Education:
- High School diploma or equivalent (GED).
- Completion of at least 100 hours from New York State approved residential Health Care Facility nursing assistant training program.
- Certifications & Licensure: -New York Nurse Aide Certification and listed in New York State RHCF Nurse Aide Registry.
