CARE NAVIGATION SPEC.
Job Title: Care Navigation Specialist
Location: Milwaukee, WI 53226
Duration: 1 year
Shift/Schedule: MondayFriday, 8:00 AM 4:30 PM (40 hours/week),
This is a hybrid role. Candidates will need to be local and okay with coming in depending on the need of the department.
Pay rate: Up to $30/hour
Job Responsibilities:

Coordinate patient intake and triage across multiple medical specialties and service lines

Navigate patients through the healthcare system and connect them with appropriate specialists and resources

Manage complex follow-up care logistics, including scheduling labs, scans, treatments, and appointments

Provide high-touch, white-glove service to high-profile patients, families, and stakeholders

Act as a liaison between patients and healthcare providers to communicate appointment details and care plans

Utilize EMR and CRM systems to maintain accurate patient records and manage care transitions

Collect and manage comprehensive patient data including medical history, financial status, and records

Coordinate with department leadership for complex or multi-service-line care plans

Perform service recovery by addressing patient concerns, advocating for patients, and restoring trust

Maintain exceptional communication, organization, and customer service standards
Requirements:
-
Minimum 4 years of related experience in intake, scheduling, referral facilitation, or care coordination across 2 or more service lines

Associate degree OR additional 2 years of relevant experience in lieu of degree

Strong logistics management and care coordination skills

Knowledge of medical terminology

Excellent computer skills, including proficiency with Microsoft Office Suite

Ability to multitask, prioritize, and work independently with strong follow-through

Exceptional communication, hospitality, and customer service skills

Strong problem-solving abilities and teamwork skills
Preferences:

Bachelors degree

Experience with EPIC electronic medical record system
