Job Summary:
The Client Occupational Therapist is responsible for coordinating and providing
restorative and rehabilitative Occupational Therapy services to participants at the Center and
at home. Occupational Therapy works closely with PACE Physician, rehabilitation staff and
other interdisciplinary team (IDT) members to maximize participant independence and safety
as well as enhance performance of ADLs. The Occupational Therapist collaborates with the
Physical Therapist to efficiently operate Client rehabilitation department and is a
member of the Client management team and facilitates collaborative relationships
across Client and with external stakeholders to promote the Client values, team
culture and mission.
-Conduct Occupational Therapy assessments to determine the Occupational Therapy
care needs, preferences and goals of the participants and actively participate in IDT
meetings to develop participant care plans.
-Deliver and document Occupational Therapy interventions in accordance with physician
orders and/or as agreed upon in the participants care plans including but not limited to
the use of devices, techniques and activities to promote muscle re-education,
perceptual-motor and cognitive function, balance, endurance and other factors affecting
the level of independence in performing ADLs.
-Supervises the Therapy Assistants including providing training, instruction and guidance
needed to implement Functional Maintenance Programs (FMP).
-Provide training and instructions to staff, participants and caregivers/families in areas of
Occupational Therapy in which they may work with participants.
-Order, track and manage maintenance for equipment.
-Remain alert to pertinent input from other team members, participants, and caregivers
and update the IDT promptly of any changes in participants condition or functional
status.
-Work with the other members of the IDT to support smooth care transitions between
settings (e.g., hospitals, skilled nursing facilities, home, etc.).
-In coordination with the Marketing Team, support enrollment of prospective participants
into the program.
-Participate in end-of-life care coordination and support.
-Manage direct reports including training, supervising and mentoring.
-Be legally authorized to practice in position location, required.
-Only act within the Product of his or her authority to practice.
-Complete all required documentation in a timely and accurate manner.
-Protect privacy and maintain confidentiality of all company procedures and information
about team members, participants, and families.
-Follow Client policies and procedures and participate in any required Quality
Improvement activities, staff training and meetings.
-Communicate regularly with Supervisor and team regarding workload and priorities.
-Timely completion of all mandated trainings and education.
-Timely completion of all mandated occupational health screenings as needed.
-Exercises flexibility in performing assignments as business needs evolve.
-Other duties as assigned.
Required Skills & Experience:
-Minimum of two years Occupational Therapy experience in a clinical setting with a frail
or elderly population.
-Highly motivated, inspirational leader who is able to innovatively engage, support and
remove barriers for the team to ensure the team consistently meet established goals.
-Occupational Therapy knowledge and skills necessary to treat frail, elderly participants
with complex needs.
-Two years supervisory experience with demonstrated ability to mentor and develop
team members.
-Ability to process accurate information in a timely manner.
-Excellent leadership, organizational and communication skills in settings with seniors,
their families and interdisciplinary team members.
-Experience leading in a data-driven organization, leveraging reports and data to
prioritize and manage people and projects.
-Excellent clinical, organizational and communication skills in settings with seniors, their
families and interdisciplinary team members.
-Ability to work independently with minimal supervision.
-Demonstrated ability to prioritize in a fast-paced environment.
-Experience and competency working with people from diverse backgrounds and
cultures.

Required Education:
-Bachelor s Degree in Occupational Therapy from Accredited Program (AOTA and ACOTE standards).
Required Certifications & Licensure:
-Initial Registration from National Board for Certification in Occupational Therapy.
