Job Title: Care Coach
Location: Sarasota and Charlotte Counties FL (75-90% local travel)
Shift: Mon-Fri; 8AM – 5PM (Strictly 40 Hours a week / No Overtime)
Duration: 3 months (Possible conversion or perm, but NOT Guaranteed. Based off performance, evaluation, and budget)

The Care Coach 1 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity.

Job Description:
The Care Coach 1 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity.
Responsibilities:
- Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long-Term Care Facilities and other care settings – 75-90% local travel
- Assesses and evaluates member's needs and requirements in order to establish a member specific care plan
- Ensures members are receiving services in the least restrictive setting in order to achieve and/or maintain optimal well-being
- Planning and implementing interventions to meet those needs
- Coordinating services, and monitoring and evaluating the case management plan against the member's personal goals
- Guides members/families towards resources appropriate for their care
- Services are driven by facilitating interactions with other payer sources, providers, interdisciplinary teams and others involved in the member’s care as appropriate and required by our comprehensive contract
- Bilingual candidates Required (English & Spanish Speaking)
- Candidates Must Be CPR Certified
- Must live in Sarasota and Charlotte Counties (Florida) (Or willing to drive to those facilities) Near Sarasota, FL
- Previous experience with electronic case note documentation and experience with documenting in multiple computer applications/systems
- Must Have Experience in Case Management, Coordinating Services, Conducting Assessments to Determine Needs
- Intermediate to advanced computer skills and experience with Microsoft Word, Excel and Outlook.
- Willing & Able to Return to Field and Travel within Locally Assigned Area (On Occasion, May Need to Travel to Surrounding Region)
- Ability to work Monday-Friday 8:30am-5:00pm
- Care Coaches with the following qualifications shall also have a minimum of two (2) years of relevant experience:
- Bachelor’s degree in social work, sociology, psychology, gerontology, or a related social services field;
- Registered nurse, licensed to practice in the State;
- Bachelor’s degree in a field other than social science; or
- Care Coaches with a master’s degree in social work, sociology, psychology, gerontology, or a related social services field may substitute experience obtained through a practicum, internship, or clinical rotation on an equivalent basis for up to one (1) year of the experience requirements.
- Care Coaches with the following qualifications shall also have a minimum of four (4) years of relevant experience: Licensed Practical Nurse, licensed to practice in the State.
- Care Coaches without the aforementioned qualifications may substitute professional human service experience on a year-for-year basis for the educational requirement.
- Case Managers without a bachelor’s degree shall have a minimum of six (6) years of relevant experience.
- 701B Certification
- Medication application (RFA) experience
- Prior experience with Medicare & Medicaid recipients
- Prior nursing home diversion or long term care case management experience
- Experience working with geriatric population
- Experience with health promotion, coaching and wellness
- Knowledge of community health and social service agencies and additional community resources
- Mileage Reimbursed for Travel (.56/Mile – First 50 Miles & Last 50 Miles – are not reimbursed)
- Client will Provide PPE
- Client will Provide ALL Equipment including Portable Wi-Fi
- 4 Weeks of Training Upon Start of Assignment
