The Intake Lead generates new patient intakes and ensures potential patients complete their initial intake appointments with a clinician. Evaluates the mental healthcare needs of clients and determines eligibility for services. QUALIFICATIONS Education Related Work Experience Required Preferred Years Required Preferred High School Graduate/GED ? ? 0 - 1 ? ? Vocational/Technical School Graduate ? ? 1 - 3 ? ? Associate Degree ? ? 3 5 ? ? Bachelor s Degree ? ? 5 8 ? ? Master s Degree ? ? 8 10 ? ? Ph.D. ? ? 10+ ? ? Other: Specialized Skills and Knowledge (List the minimum required specific knowledge and skills necessary to perform this job. (e.g., basic accounting principles, computer programming, spreadsheets, specific healthcare processes, etc. 1. Proficient in software applications 4. Ability to write detailed correspondence 2. Ability to read and comprehend moderate to complex instructions and correspondence 5. Excellent verbal and written communications skills 3. Ability to effectively present information in one-on-one and small group situations to patients and their families, facility leadership, employees clients, providers, vendors, etc. 6. Ability to listen, empathize and offer constructive guidance. Essential Job Duties/Responsibilities % Of Time 1. Responds to intake calls and completes admission paperwork. Assessments will be completed by RN, LPC or LMSW. 20% 2. Conducts interviews individually or in families to assess their situation and determine types of services required. 15% 3. Tracks all inquiries and resolves issues as it pertains to the intake process. 10% 4. Collects data to help with the referral process. Follows up on referrals by contacting referral sources to collect data. 10% 5. Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities. 20% 6. Reviews newly admitted patient accounts for eligibility and authorization daily. 10% 7. Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships. 5% 8. Participates as an active member of the interdisciplinary team. 5% 9. Ensures that all Intake forms are properly completed and filed. 5% 10. Performs other duties as assigned/required by this position. 5%
