JOB SUMMARY Under the supervision of the Assistant Director Cardiology: Assesses, plans, implements, evaluates, and supervises individualized care and group exercise sessions in The Cardiac Rehabilitation Department. Provides patient education in the areas of cardiac disease (including heart failure, coronary artery disease and risk factor management, post- MI, and post surgery), diabetes, and other chronic diseases. She/he will individualize patient care based upon the age appropriate and developmental needs and will accept responsibility for the direction of coworkers in the implementation of the plan of care. Collaborates with physicians, patient/families and members of the health care team in delivering a plan of care. Utilizes positive communication skills. Should be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her unit. Performs other duties as assigned

PRIMARY JOB DUTIES
1. Completes Intake assessment of patient with Cardiac Rehab Staff RN and documents appropriately into
patient record.
2. Develops a plan of care based upon the assessment of patient, age appropriate and developmental needs
of the patient with the RN who performed the nursing assessment.
3. Collaborates with physicians and other health team members in developing a plan of care.
4. Provides and coordinates the plan of care.
5. Initiates, manages, and participates in discharge planning. Initiates, manages, and participates in
patient/family teaching
6. Provides diabetes self management education (DSME) which primarily targets patients at risk for
diabetes, Type I, and Type II diabetics.
7. Evidenced based standards as recommended by the American Diabetes Association (ADA) and
American Association of Diabetes Educators (AADE) are utilized in providing diabetes education.
8. Upon initial intake/orientation of the diabetes patient, the exercise physiologist (EP) will conduct an
individual assessment of patient and collaborate with patient in plan/strategy development for diabetes
self management.
9. The EP will provide educational interventions (including
psychosocial, nutritional and exercise domains) aimed at patient
achieving diabetes self management.
10. The EP will periodically re-assess diabetes self-management results and goals every 30 days, or more
frequently as needed.
11. Upon completion of the individual patients cardiac rehab program, the EP will collaborate with patient
on recommendations and future plans for diabetes self management.
12. The EP will document all diabetes education on the patients Individual Treatment Plan under the
education category.
13. Demonstrates implementation skills in accordance with Policies and Procedures.
14. Evaluates and updates patients response to plan of care.
15. Maintains professional standards by participating in inservices, committees, and performance
improvement activities, etc.
16. Facilitates physiological, psychosocial, and spiritual care of the patient.
17. Utilizes positive communication skills to affect conflict resolution and teamwork
18. Contributes to cost effective patient care by monitoring use of time, equipment, and supplies.
19. Complies with safety policies and procedures in order to provide a safe environment for employees,
patients, and visitors.
20. Demonstrates necessary flexibility in regard to changes in Hospital or unit activities.
21. Should be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age
of the patient served on his/her unit.
22. Actively participate in achieving departmental goals.
23. Responsible for exercise prescription assessment.
24. Interprets heart rate and blood pressure responses during exercise therapy sessions.
25. Recognizes symptomatology for cardiac, respiratory and musculoskeletal systems and reports to RN.
26. Responsible for being the Code Blue Team participant and will work under the direction of the Code
Blue Team Leader and/or Cardiac Rehab Staff RNs.
27. Responsible for accurate and timely communication to other staff, patients, and physicians regarding the
patients program, progress, orders and treatment.
28. Completes all required documentation including progress notes on the Q-Tel RMS System.
29. Agrees to flex up hours during other staff vacations, attendance to education programs, or meetings due
to nature of small closed unit.
30. Cardiac Rehab Exercise Physiologist is a role model for patients.
