Reports To: Family Health and Wellness Quality Improvement Director
Office Location: Andrew Johnson Tower, Nashville, TN (travel to office approx. once every two months, non-reimbursable)

About the Role:
The Tennessee Department of Health is advancing Health-Related Social Needs (HRSNs) screening and referral processes to improve access to essential social and economic services impacting health outcomes. Partnering with TennCare (Tennessee’s Medicaid agency), this initiative implements a Closed-Loop Referral System (CLRS) via the FindHelp platform across three key programs:

About the Role:
The Tennessee Department of Health is advancing Health-Related Social Needs (HRSNs) screening and referral processes to improve access to essential social and economic services impacting health outcomes. Partnering with TennCare (Tennessee’s Medicaid agency), this initiative implements a Closed-Loop Referral System (CLRS) via the FindHelp platform across three key programs:
- Community Health Access and Navigation in Tennessee (CHANT)
- Evidence-Based Home Visiting (EBHV)
- Viral Hepatitis
Key Responsibilities:
- Lead the implementation and coordination of the CLRS initiative across CHANT, EBHV, and Viral Hepatitis programs.
- Serve as the primary liaison with FindHelp for collaboration and problem-solving.
- Work with TennCare, IT, and data governance teams to ensure smooth system integration.
- Develop and deliver training programs for staff and community partners on platform usage.
- Collaborate with community engagement staff to enhance platform adoption strategies.
- Monitor referral data and platform analytics to drive continuous improvement.
- Facilitate regular stakeholder meetings to track progress and address challenges.
- Identify barriers to system adoption, especially in resource-limited areas, and implement solutions.
- Support integration of the referral system into existing workflows.
- Promote collaboration among state and local agencies to strengthen referral networks.
- Contribute to other Family Health and Wellness initiatives as needed.
- Oversee workflow analysis, IT configurations, and system integrations related to the project.
Qualifications:
- Bachelor’s degree from an accredited institution with a minimum of 3 years’ experience in public health, program implementation, or project management.
- Master’s degree in Public Health (MPH) or related field strongly preferred.
- Demonstrated ability managing multi-stakeholder projects in health or human services sectors.
- Excellent organizational and coordination skills to manage complex projects with diverse partners.
- Detail-oriented with strong tracking and milestone management abilities.
- Skilled in team engagement and fostering collaborative participation.
- Experience in program coordination, community engagement, or care coordination.
- Proficient in data analysis and reporting to support program monitoring.
- Strong computer skills with Microsoft Office Suite (Word, Excel, Outlook, Teams) and familiarity with project management tools like Asana.
- Experience with digital referral platforms, electronic health records, or case management systems is a plus.
