Position Summary:
- We are seeking a self-motivated, detail-oriented, and tech-savvy Case Management Coordinator to join our dynamic team.
- This role offers autonomy and flexibility while coordinating care for members in the Long-Term Care/Comprehensive Medicaid Program.
- Conduct face-to-face and telephonic comprehensive assessments to evaluate members' medical, functional, and social needs.
- Develop, implement, and monitor individualized care plans using care management tools and clinical judgment.
Required Qualifications:
- Must be bilingual (Spanish/English).
- Minimum 1 year of case management experience.
- Bachelor’s degree required (Social Work or related human services field preferred).
Preferred Qualifications:
- Case Management Certification (e.g., CCM)
- Experience in long-term care settings (e.g., ALFs, SNFs, home care)
