Duties:
MD ORDERS FOR ALL MSW - PCLS CONSULTS
Psychiatrist/Providers do the initial screens for all PCLS consults to determine if Holds are needed/Holds Continuing or if case is CONSULT ONLY for Psychiatrist for medication assistance. (NOTE: Consult Only cases are generally only followed by Psych MD unless otherwise specified by Psych MD)PCLS Provider determines PCLS SW involvement based on patient (pt) remaining on Legal Hold status. If case is a CONSULT ONLY or HOLD IS REMOVED at the same time as Psych MD screens the pt, the PCLS SW will not be involved for ongoing care since pt is not a psychiatric pt.
PCLS PATIENTS ON LEGAL HOLDS:(5150/5585, 5250, TCON or PCON)
If pt is on a legal hold status, the PCLS SW will do Initial Assessment of pt. They will complete the Initial Assessment form from the EPIC SW Navigator tab.
o Social Determinants of Health (SDOH) must be completed for every admitted patient and is built into the SW Initial Assessment.
Initial Assessment will be same as Unit SW Initial Assessment EPIC form for downtime procedure. The PCLS SW will evaluate pt s mental status, living situation, support system, drug hx, mental heath hx, physical functional status in community, financial status, discharge planning needs, and make recommendation for follow-up. (ie. MediCal screening, BH services, SUD or placement).
If there is an EPIC SW Consult related to a PCLS issue that is being address, the PCLS SW will complete the consult and assess/respond.
PCLS SW is expected to communicate with Nursing Management on the unit of pt s who have legal holds.
o PCLS will provide current status and possible expiration of Legal Hold dates.
PCLS SW will communicate with Unit SW/CM the potential Discharge planning needs and assist with planning for psychiatric related services and legal hold expiration and/or legal status transitions.
PCLS SW staff expected to maintain contact with agencies/families for collateral fact gathering & if needed to secure discharge arrangements that are not medically related (i.e. room and board, homeless shelter, transportation).
PCLS SW will identify likely psychiatric outpatient follow up that may be needed when discharged. PCLS SWs see the legal hold pts on 5585/5150/5250 daily and LPS Conserved pts 3 x a week.
PCLS SW will make attempts to transfer pt s to ITF or other Riverside County LPS approved Facilities when medically cleared. However, due to limited bed availability, will also continue to work on alternative Discharge Plans when Psychiatrically/Physically cleared.
PCLS SWs will continue to monitor cases through the duration of legal holds and complete appropriate advisements as they transition from one level to another (i.e 5150 to 5250 to TCON). PCLS PATIENTS THAT ARE GD & ON A LEGAL HOLD:
PCLS SWs must monitor pt s potential for conservatorship based on GD status for psychiatric needs for LPS Conservatorship. It MUST be timely during first week of admission to ensure appropriate referrals to Public Client are made.
Education:
This is the pay range that RightSourcing (a part of Magnit)
reasonably expects to pay someone for this position, however, as a supplier your expected pay range may vary and/or include certain benefits like: Stipends (for clinical traveler workers only), Medical, Dental, Vision, 401K [include any compulsory benefits such as commissions, incentive bonuses, etc. if applicable].
CERTIFICATION REQUIRED:must have either an interim BBS license (AMFT/ACSW/APCC)

Minimum two year s of experience in clinical setting required, will assist with our current SW staff shortage. Previous counseling experience preferred. Able to read; write legibly; speak in English with professional quality; use computer, printer, and software programs necessary to the position; operate/troubleshoot basic office equipment; type 30 words per minute without errors. Able to relate and communicate positively, effectively, and professionally with others; provide leadership and supervision; follow policies consistently; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to positively and effectively manage complex interpersonal relations with patients and staff. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; work independently and with minimal supervision; perform basic math functions; manage multiple assignments effectively; compose written material; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Works independently without direct supervision. Able to distinguish colors as necessary; write, hear, read and speak sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with workplace; see adequately to read computer screens and written documents necessary to the position.
