Job Description:

Key Responsibilities:
- Review and interpret physician orders for accuracy and appropriateness.
- Provide drug information and education to healthcare professionals and patients.
- Monitor patient responses to prescribed drug treatments.
- Collaborate with the healthcare team to develop and implement individualized medication plans.
- Evaluate potential drug interactions, contraindications, and allergies.
- Ensure compliance with federal and state pharmacy laws and hospital policies.
- Maintain accurate and detailed medication records.
- Participate in quality improvement and medication safety initiatives.
Qualifications:
- Doctor of Pharmacy (PharmD) degree from an accredited college or university.
- Active and unrestricted Pharmacist license in the state of California.
- Minimum of 1-2 years of recent clinical pharmacy experience (hospital setting required).
- Epic experience required.
