Length: 3 Months+ (with possible extension)

Facility: Hospital setting
Schedule: Monday–Friday | 5x8s | 8:00 AM – 4:30 PM
Pay Rate/hr: $53
Weekly gross: $2120 (all-inclusive)
Weekly Per diem/non-taxable: $954
Pay Rate/hr on W2: $29.15
On-Call: Possible

Position Overview:
We are seeking a compassionate and experienced Social Worker (MSW I) to join a comprehensive care management team in Fort Worth, TX. In this role, you will support patients and families as they navigate emotional, social, and medical challenges, while collaborating closely with interdisciplinary healthcare teams to ensure continuity of care and positive patient outcomes.
This is an excellent opportunity for a motivated social worker looking for a travel assignment with potential for permanent placement.

Key Responsibilities

• Conduct and document comprehensive psychosocial assessments to evaluate patient needs.
• Develop, implement, and evaluate individualized treatment and care plans.
• Provide patient and family education, counseling, crisis intervention, and bereavement support.
• Coordinate services including discharge planning, referrals, advocacy, and community resource linkage.
• Collaborate with interdisciplinary healthcare teams to ensure integrated, patient-centered care.
• Serve as a liaison between patients, families, and community organizations.
• Prepare professional documentation including reports, letters, and case summaries.
• Participate in performance improvement initiatives, data collection, and continuing education efforts.

Required Qualifications:

• Master of Social Work (MSW)
• LMSW (Texas license required)
• BLS Certification (American Heart Association only)
• Minimum 1 year of experience in a healthcare or hospital setting.
• Strong communication, assessment, and problem-solving skills.
