Shift: Other – Schedule C
Schedule: 3 Days Per Week, 36 Hours Per Week, 12-Hour Shifts
• Wednesday: 8:00 PM – 8:30 AM
• Saturday: 8:30 AM – 8:00 PM
• Sunday: 8:30 AM – 8:00 PM
• No on-call
Pay Range: $70-$75/hr.
Required Skills & Experience:
-
A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State – Required
-
For licenses issued on or after July 1, 2020: A Bachelor’s of Science degree in Nursing or higher (or other nursing degree, as identified by NYS Education Department) – Required

Valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA) – Required
Preferred Skills & Experience:
-
Experience in healthcare settings caring for people experiencing homelessness, mental illness (including substance use disorders), or justice involvement – Strongly Preferred
-
Wound care experience – Preferred
-
Experience with naloxone (Narcan) administration and patient education – Preferred
-
Excellent written and verbal communication skills – Preferred
-
Sensitivity to the needs of a culturally and linguistically diverse patient population – Preferred
Licensure & Certifications:

New York State RN License – Required

BLS – Required (American Heart Association only)
Job Summary:
Client’s Bridge to Home (BTH) program provides treatment, social and professional skill-building, and temporary housing to patients with serious mental illness (SMI) who are ready for hospital discharge but lack access to stable housing.
Essential Duties and Responsibilities:

Assess patients through interviews, chart reviews, and referrals

Educate patients based on diagnosis and identified needs

Administer medications and vaccines
-
Initiate nursing interventions based on clinical assessments
-
Instruct patients/families on treatment plans and medications
-
Recognize and address contributing factors (e.g., socioeconomic, cultural, language barriers)
-
Ensure patients understand their medications and maintain accurate med lists
-
Coordinate response to medical and psychiatric emergencies
-
Schedule and conduct follow-up visits and specialty referrals
-
Perform triage and take vital signs
-
Document evaluations and interventions in EMR
-
Demonstrate proficiency with nebulizers and metered dose inhalers
-
Apply harm reduction and patient-centered care principles
-
Work collaboratively within interdisciplinary care teams
-
Attend monthly care coordination meetings, staff meetings, and trainings
-
Provide outreach in various settings (e.g., shelters, street outreach) as needed
