Job Title: Patient Care Coordinator / Case Manager
Location: San Jose, CA - 95119
Duration: 3+ Months (Possible Extension)
Shift: 08:00 AM – 04:30 PM

Job Summary:
We are seeking a Patient Care Coordinator / Case Manager to support care coordination and patient management activities within a clinical setting. The ideal candidate will ensure seamless communication between patients, providers, and care teams while maintaining high standards of patient service and operational efficiency.
Key Responsibilities:
- Coordinate patient care activities and ensure timely follow-up with providers and care teams.
- Manage patient cases by monitoring treatment plans, appointments, and care progress.
- Serve as the primary point of contact for patients regarding care coordination and scheduling.
- Communicate with physicians, nurses, and healthcare staff to ensure continuity of care.
- Maintain accurate patient records and documentation in the electronic medical record (EMR) system.
- Assist patients with navigating healthcare services, referrals, and resources.
- Ensure compliance with healthcare regulations, policies, and confidentiality standards.
Requirements:
- Experience in patient care coordination, case management, or healthcare administration.
- Strong communication and interpersonal skills.
- Experience working with EMR/EHR systems.
- Ability to manage multiple cases and prioritize tasks effectively.
