Key Responsibilities:
- Assess, plan, implement, and coordinate case management activities to address members’ medical needs and promote overall wellness.
- Develop proactive care plans to improve both short- and long-term outcomes through integrated services.
- Coordinate and monitor assigned care plan activities, tracking progress toward goals.
- Conduct multidisciplinary case reviews to achieve optimal results.
Required Qualifications:
- Current, unrestricted Registered Nurse (RN) license in the state of Texas.
- Minimum 3+ years clinical practice experience.
- At least 1 year of case management experience.
- Comfortable with extensive local driving (up to 1 hour per trip).
Preferred Skills:
- Strong communication and negotiation skills.
- Experience in managed care, discharge planning, or utilization review.
- Proficient in motivational interviewing and patient engagement strategies.
