Healthcare Consultant II - Texas Star Kids

Position Summary:
The Nurse Case Manager uses a collaborative process of assessment, planning, facilitation, and advocacy to meet members’ health needs and optimize outcomes. This role involves both telephonic and face-to-face interactions, requiring strong clinical judgment, case management expertise, and excellent communication skills.
 
Key Responsibilities:

• Assess, plan, implement, and coordinate case management activities to address members’ medical needs and promote overall wellness.
• Develop proactive care plans to improve both short- and long-term outcomes through integrated services.
• Coordinate and monitor assigned care plan activities, tracking progress toward goals.
• Conduct multidisciplinary case reviews to achieve optimal results.

Required Qualifications:

• Current, unrestricted Registered Nurse (RN) license in the state of Texas.
• Minimum 3+ years clinical practice experience.
• At least 1 year of case management experience.
• Comfortable with extensive local driving (up to 1 hour per trip).

Preferred Skills:

• Strong communication and negotiation skills.
• Experience in managed care, discharge planning, or utilization review.
• Proficient in motivational interviewing and patient engagement strategies.
