Shift: 3rd Shift
Schedule: 3 Days Per Week, 36 Hours Per Week, 12-Hour Shifts
Additional Scheduling Notes: Schedule A: Sunday, Monday, Tuesday – 8:00 PM to 8:30 AM; No on-call
Pay Range: $70-$75/hr.
Required Skills & Experience
-
Experience or prior work in healthcare settings caring for people with experiences of homelessness, mental illness (including substance use disorders), or justice involvement – Preferred, but strong desire to learn and care for this population is expected
Preferred Skills & Experience

Wound care experience – Preferred

Experience with naloxone (Narcan) administration and patient education – Preferred

Excellent written and verbal communication skills – Preferred

Exhibits sensitivity to the needs of a culturally and linguistically diverse patient population – Preferred
Education

A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State – Required

For individuals issued a registered professional nursing license on or after July 1, 2020: A Bachelor’s of Science degree in Nursing or higher (or other nursing degree, as identified by NYS Education Department) – Required

Job Summary
Client will offer its Bridge to Home (BTH) program, providing treatment, social and professional skill building, and temporary housing to patients with serious mental illness (SMI) who are ready for discharge from the hospital but do not have immediate access to stable housing.
Essential Duties and Responsibilities:

Assess patients based on interviews, chart reviews, and/or referrals

Educate and teach patients based on diagnosis and need
-
Administer medications as needed
-
Initiate appropriate nursing interventions, including:
-
Educating patients/family on treatment, medications, procedures, and follow-ups
-
Identifying underlying/contributing factors (socioeconomic, cultural, physical, language)
-

Maintain accurate medication lists and assist patients in understanding prescriptions
-
Coordinate response to medical and psychiatric emergencies
-
Schedule nurse follow-up visits (e.g., wound care, medication education)
-
Assist patients in accessing specialty services
-
Triage patients and perform vital signs
-
Document all observations and interventions in EMR or forms
-
Participate in preventive health measures, including vaccine administration
-
Administer nebulizer and metered dose inhalation therapies
-
Apply harm reduction and patient-centered approaches
-
Provide culturally and linguistically sensitive care
-
Collaborate with interdisciplinary team for patient-centered care
-
Attend monthly coordination meetings, staff meetings, and training sessions

Conduct outreach in community settings (e.g., shelters, streets), if necessary
