Job Title: Social Worker
Location: Washington, DC 20037
Duration: 3 months+ (possible extension)

Unit: Rehabilitation Unit
Shift: 8:30 AM – 5:00 PM
Schedule: Monday – Friday
Note: Candidate must live at least 50 miles away.
 
The Payrate Information:
Overall Weekly Pay: $2,123.25
Weekly Stipend: $1,163.25
W2 hourly Pay Rate: $24
 
Requirements:

• Minimum 1 year of hospital case management experience, particularly in discharge planning.
• Familiarity with Cerner EMR system

• LICSW certification and an active DC state Social Work license – required

 
Job Summary:

• Provides professional social work support to patients and their families in accordance with licensure, training, and regulatory standards. This role is responsible for coordinating patient services during and after admission to the acute rehabilitation unit.
• Key responsibilities include discharge planning, care coordination, and counseling.
• Ensure patients transition smoothly through the continuum of care while promoting efficient use of healthcare resources.
• The position also requires adaptability to changing priorities and assignments.
