Job Description: JOB DESCRIPTION Associate Member Health Accessor
Job Summary
•** Services is hiring for a Member Health Assessor in our Long Beach, CA
office! You are able to set your hours, however it does require that you work from our Long Beach
office.
This is an entry level role that is heavily administrative. You will be printing documents, folding, then
stuffing envelopes. You will be ensuring that Company correspondence is ready for the mail.
Basic knowledge of MS Word and Excel is also required.
KNOWLEDGE/SKILLS/ABILITIES
Utilizes a collaborative process of assessment, planning, implementation and evaluation
to engage, educate, and promote members' decisions related to achieving and
maintaining optimal health status.
May request RN input for member care planning.
Works with members with the least complex clinical needs.
Expected to meet daily production standards in terms of member outreach.
Utilizes a web-based software system to document all assessments, activities and
education provided.
Develops a problem list and individualized care plan using program protocols based on
the members needs and preferences.
Conducts telephonic education to promote self-management strategies for all applicable
conditions.
JOB QUALIFICATIONS
Required Education
HS Diploma/GED
Required Experience
Minimum 1 year experience working with underserved or special needs populations, with varied
health, economic and educational circumstances.
Required License, Certification, Association
Must have valid driver's license with good driving record and be able to drive within applicable state
or locality with reliable transportation.
Preferred Education
Associate Degree in health care related field.
Preferred Experience
Bilingual based on community need.
Familiarity with healthcare systems a plus.
Knowledge of community-specific culture.
Experience demonstrating knowledge of health care basics, community resources,
communications (verbal and written), Individual and family advocacy, social services,
health education and service skills and responsibilities.
